Consists of two main areas: case management and cardiac support.

• Case management is an extensive follow-up program for those with heart failure or cardiovascular disease. The goal of this program is to get patients to better understand and self manage their disease.
  
  
• Cardiac support provides a monthly support group for those who have suffered from a cardiac injury. This program is open to both patients and the public.
