Geriatric Assessments

A geriatric assessment is a one-time visit with a team of healthcare providers who are specialists in the health and wellness of older adults. The first part of clinic is a multi-faceted evaluation by a team of healthcare providers. The second part of the clinic is an extended discussion with the participant, their family, and our team about what to expect from aging process and planning for the future. The purpose of an assessment is to identify ways to optimize the participant’s ability to enjoy good health, improve overall quality of life, reduce the need for hospitalization and/or institutionalization, and enable the participant to live as independently as possible for as long as possible.

The assessment consists of the following:

  1. A referral from the participant’s primary care physician. This helps ensure that the Clinic team is working with your physician regarding your medical and care needs.

  2. An examination of the participant’s current status. This includes physical health, medication(s), cognition, activities of daily living, strength, balance, psycho-social status, advance care planning, and support systems.

  3. Identification of current and/or anticipated problems. This information is gathered from the self-assessments, clinic findings, and conversations/interviews with the participant and family members.

  4. The development of a report. A comprehensive report will be sent to the referring physician, which addresses the identified problems, suggested interventions or actions, and guidance toward resources and support services.

  5. Further follow-up if necessary. In some cases a follow-up visit to the Geriatric Assessment Clinic may be warranted and/or recommendations to consult with other specialists may be given. If follow-up is necessary, it will be part of your comprehensive report.
