Mid Coast–Parkview Health offers several convenient, secure ways for you to access your medical records.
MyChart Patient Portal
Mid Coast Hospital, Mid Coast Medical Group, and CHANS Home Health & Hospice patients are able to sign up for the MaineHealth MyChart Patient Portal. MyChart replaces the existing Mid Coast Medical Group Patient Portal and Mid Coast Hospital Patient Portals.
MyChart is a secure, online tool that lets you connect with your MaineHealth provider and manage your health information online. With MyChart, you will be able to renew prescriptions, schedule new appointments, view your lab and test results, message your provider’s office, keep track of your medications, allergies, and pay your bill. MyChart is free and easy to use, and you will have access 24 hours a day from your computer, tablet, iPhone, or Android.

You may also request a copy of your medical records at any time by downloading and completing the MaineHealth Authorization to Release and Disclose Protected Health Information (PHI) form. Once completed, please mail the form to:

HealthInfoNet
Mid Coast–Parkview Health participates in HealthInfoNet, a statewide computer system that helps caregivers provide better care by combining information from all participating caregivers into a single electronic record. This includes information about your medications, allergies, test results, and health problems.
You may opt out of HealthInfoNet using the form in this PDF.
Note: Mid Coast–Parkview Health supports patients having access to their health information. Beyond the Patient Portal, we also support sharing of health information via applications that use an Application Programming Interface (API), which many mobile device applications use.
